Doctors classify gynecomastia into four grades, based on the amount of excess tissue and skin:

How is Gynecomastia surgery performed?
Many surgeons use techniques typically used for female breast surgery, but Dr. Rodriguez believes this is not the best approach for male patients. Male breast reduction should aim to create a flat, non-projecting chest, which is the opposite of the objective in female breast surgery.
Liposuction Alone
The surgical technique used to correct gynecomastia depends on the amount of fat and breast tissue that needs to be removed, as well as the quality of the skin and the position of the nipples. For men with good skin tone and minimal nipple sagging, liposuction may provide excellent results. In some cases, over a pound of tissue can be removed from each side.
Liposuction and Tissue Removal
The most common gynecomastia surgery technique combines liposuction with tissue removal. For larger breasts or sagging skin, skin excision (mastopexy) may also be required.
- Local anesthesia: If a small amount of tissue is removed, Dr. Rodriguez performs the surgery under local anesthesia.
- IV sedation: If more than 300cc of tissue is removed, Dr. Rodriguez recommends IV sedation to keep the patient comfortable. IV sedation is also used when skin excision is required.

What are the preoperative requirements?
Pre-op requirements depend on whether the procedure is done under local anesthesia or IV sedation.
Local Anesthesia
- You don't need pre-op clearance, and you can eat a light breakfast before surgery.
- You can eat a light breakfast before surgery.
IV Sedation
- Obtain medical clearance, including a History & Physical and a CBC blood test, 30 days before surgery.
- Avoid eating or drinking after midnight before surgery.
Additional Pre-op Requirements
- Your BMI must be under 35.
- Stop smoking at least two weeks before surgery.
- Stop taking multivitamins, herbal supplements, and aspirin two weeks prior to surgery.
- Fill all prescriptions before the surgery date.
What is Gynecomastia Post-Op recovery like?
Post-operative recovery is usually smooth, with minimal bruising, although some swelling and soreness can occur. Recovery instructions include:
- Rest for the first 2-3 days and drink plenty of fluids.
- Contact Dr. Rodriguez if you experience a fever higher than 101°F during the first week.
- Expect mild chest discomfort for the first 2-3 weeks.
- Wear the medical-grade compression garment for the first five days continuously.
- No showering for the first five days.
- Return to work after 6-7 days.
- Most regular activities can be resumed after two weeks.
- Continue wearing the compression garment for up to four weeks.
- Avoid strenuous chest exercises for four weeks.
- Follow-up visits at 1 month, 4 months, and 1 year.
What are the risks and possible complications?
Gynecomastia surgery, like all procedures, has some risks. These include:
- Hematoma
- Seroma
- Fat necrosis
- Infection
- Changes in nipple or breast sensitivity
- Asymmetry
- Scarring
- Regrowth of breast tissue
